Please download and print the packet. Have mom sign the first three pages. Keep a copy for yourself and then provide a copy to us by emailing it to rachel@kirsh.com.
Please staple the rest of the pages (Birth Mother’s Background Form) and provide that to mom to work on. Please ask her to have the packet completed by the time we arrive at the hospital to have her sign the consents to adoption.
